Chopping off heads with my flash

Frequently, when I use a flash with my 35mm SRL the top half of the image is blacked out. Is this an equipment problem or am I using the wrong setting on the flash (or aperture)?

You're setting a shutter speed faster than your camera's flash sync speed, or x-sync. On older manual cameras this maximum shutter speed is marked in red or with an x, eg. x60. On newer electronic cameras there is no indication on the camera and you'll have to consult your user manual.
