If I'm looking at my mushroom book right it is called Fly Amanita (Amanita muscaria). The "splotches" are a feature of the mushroom. It is highly poisonous---but rather beautiful for a mushroom. I've seen it once or twice in the woods here on the Olympic Peninsula, but this year some of my neighbors seem to have colonies of these in their yards. As they get older the tops flatten out and they're not nearly so neat looking. They say it's called Fly Amanita because in Europe, years ago, an extract was supposedly made from it that killed flies. #25760
